ABSTRACT

Resumo Introdução: A infecção por Echinococcus ou hidatidose constitui-se em uma zoonose causada pelo parasita Echinococcus granulosus. Apresenta distribuição mundial, sendo Portugal considerado endêmico. A doença hidática do rim é rara, ocorrendo em apenas 2% dos casos. Caso Clínico: Adolescente de 14 anos, do sexo masculino, com doença hidática do rim. O diagnóstico foi efetuado por exames de imagem e por teste sorológico específico. Foi instituída terapêutica com albendazol durante quatro ciclos de quatro semanas cada, tendo-se verificado diminuição significativa do tamanho do cisto e negatividade dos anticorpos específicos. Discussão: Embora a terapêutica recomendada seja a excisão cirúrgica, estudos demonstram que a terapêutica médica pode ser uma alternativa a considerar, permitindo a preservação do órgão.


Abstract Introduction: Hydatid disease is a zoonosis caused by the parasite Echinococcus granulosus. It has a worldwide distribution, being endemic in Portugal. Hydatid disease of the urinary tract is rare, occurring in less than 2% of all cases. Case report: 14-year-old male with hydatid disease of the kidney. The diagnosis was established by imaging studies and specific serologic testing. Treatment consisted of four 4-week cycles of albendazol and resulted in a progressive reduction of the cyst as well as in a negative specific serology. Discussion: Even though the recommended treatment is surgical excision, studies have demonstrated that medical treatment may be an alternative allowing for organ preservation.

ABSTRACT

Leishmaniasis is an infectious disease caused by protozoa of the genus Leishmania transmitted by insects of the genus Lutzomyia sp. or Phlebotomus sp. The main syndromes are cutaneous leishmaniasis, mucocutaneous leishmaniasis, visceral leishmaniasis (kala-azar) and post-kala-azar dermal leishmaniasis. This article reviews kidney involvement in cutaneous and visceral leishmaniasis, highlighting the aspects of their pathophysiology, clinical manifestations, histopathological findings, outcome and treatment.

ABSTRACT

This study reports a case of parasitism by Dioctophyme renale in a supernumerary kidney and abdominal cavity of a female cat in Brazil. The three-year-old cat of indeterminate breed presented abdominal distension and was taken to the University of Contestado Veterinary Hospital in Canoinhas, state of Santa Catarina, since the owner suspected pregnancy. An ultrasound scan did not confirm pregnancy but revealed parasitism in the kidney. This case is worth reporting because domestic cats are rarely hosts of this nematode species.


Relata-se um caso de parasitismo por Dioctophyme renale em rim supranumerário e em cavidade abdominal de um gato do Brasil. Uma fêmea adulta, não castrada, sem raça definida e com aproximadamente três anos de idade, foi encaminhada ao Hospital Veterinário da Universidade do Contestado, em Canoinhas-SC, com aumento de volume abdominal e o proprietário suspeitava de gestação. Após ultrassonografia foi descartada a gestação e foi detectado parasitismo em rim. Como gatos domésticos são considerados hospedeiros incomuns desse parasita, relata-se o presente caso.

ABSTRACT

Hydatidosis is a parasitic endemic disease in Tunisia. The liver and lung are the most common sites of involvement; however, it can develop anywhere in the body. The aim of the present study was to analyse the epidemiological features of extrapulmonary hydatid cysts and compare our results with those reported in literature. Aretrospective study of 265 extrapulmonary hydatid cysts collected over the 18-year period from 1990 to 2007 was undertaken. There were 101 male and 164 female patients [sex ratio M/F= 0, 61] ranging in age from 2 to 84 years [mean age = 38, 7]. In our series, hydatid cysts involved mainly the kidney [24, 1%], the central nervous system [22, 6%], the liver [19, 6%] and the spleen [11, 3%]. The other less frequent sites included the peritoneum [n = 9], heart [n = 9], bone [n = 6], adrenal gland [n = 4], epiploon [n = 4], orbit [n = 4], ovary [n = 3], prostate [n = 2], bladder [n = 2], breast [n = 2], Douglas' cul-de-sac [n = 2], diaphragm [n = 1], testis [n = 1], broad ligament [n =1], mediastinum [n = 1], nasal cavity [n = 1], soft tissue [n = 1], abdominal wall [n = 1], parotid gland [n = 1], psoas muscle [n =1], synovia [n = 1], thymus [n =1] et le pancreas [n = 1]. In contrast to literature, our results show that hydatid cysts of the kidney and of the central nervous system are more frequent than hepatic location which occupies the 3rd rank

ABSTRACT

Contrary to the popular perception, hydatosis does not remain restricted to certain geographical locales but is a global health concern, particularly in the rural countryside. Although liver and lung involvement account for the majority of the lesions, primary breast, bone, kidney and soft tissue hydatosis with features of cystic echinococcosis are extremely rare. We report four such rare cases of cystic echinococcosis which were diagnosed and treated in our institute.

ABSTRACT

Hydatid cyst can occur in any part of the body. Isolated involvement of viscera other than liver and lung is rare In this report we present a case of seven years old male child who had an isolated involvement of left kidney with hydatid cyst. CT scan showed multiple cysts of left kidney with no renal function. His indirect hemagglutination [IHA] test showed a value of 1:8192. He was initially put on albendazole to which he showed partial response. At surgery multiple hydatid cysts were removed from kidney. Post operative radio isotope renal scan revealed the function of 15%

ABSTRACT

The kidney trematode Paratanaisia bragai is reported for the first time parasitizing the ring-necked pheasant (Phasianus colchicus L., 1758) and the pathological alterations associated to the parasitism are referred on the basis of 50 specimens of this bird from backyard flocks in 11 counties of the state of Rio de Janeiro, Brazil after clinical examination, necropsies, and histopathological analysis. The counting of the kidney flukes was based on worms recovered from one of the kidneys, since the other was fixed in 10 percent formalin and then routinely processed for histopathological procedures. The prevalence of P. bragai was of 22 percent, with a mean intensity of 44.3, mean abundance of 9.7, and range of infection of 3-153. Parasitized birds did not present with clinical signs and kidney gross lesions. Microscopic lesions were mild and characterized by dilatation of the renal medullary collecting ducts, occasional flattening of the lining epithelium of the ducts and inflammatory reaction of variable intensity with granulocytes around the ureter branches and medullary collecting ducts. The severity and pattern of the microscopic lesions seem not to be associated to the size of the worm burden and could be related to the mechanic action of the parasites, without traumatism, in despite of the presence of the tegumentar spines in specimens of P. bragai.

ABSTRACT

Envolvimento da funçäo renal em pacientes com leishmaniose visceral (calazar). Apresenta-se estudo prospectivo de 11 pacientes com LV, com o intuito de verificar as repercussöes clínico-laboratoriais da funçäo renal na doença. Realizou-se análises laboratoriais das amostras de sangue e urina, colhidas logo após confirmaçäo diagnóstica, através do encontro de leishmanias no aspirado de médula óssea. Dois (18 por cento) pacientes apresentaram complicaçöes associadas a LV. Cinco (45,4 por cento) apresentaram hematúria macroscópica e em um caso (No.9) manifestaçöes clínicas compatíveis com síndrome nefrítica aguda. Os resultados dos exames de urina mostraram: proteinúria em 10 (90,9 por cento) pacientes, hematúria 7 (63,6 por cento) e leucocitúria em 6 (54,5 por cento) casos. Nove (81,8 por cento) pacientes apresentaram níveis elevados de microalbuminúria caracterizando lesäo glomerular. A presença de tubulopatia proximal medida através da proteína ligadora de retinol, foi observada em 5 (45,4 por cento) casos. Concluiu-se que o envolvimento renal se fez presente na maioria dos pacientes, contribuindo para a gravidade da doença

ABSTRACT

We report a case of echinococcal disease affecting the brain and the kidney. The case is unusual because of multiplicity of the intracerebral lesion and sparing of the liver and lungs. The intracerebral lesion was presented with epileptic convulsions. This was successfully treated surgically. Subsequently renal echinococcal cysts were identified. These were treated by partial nephrectomy due to the location of the cysts. The patient is disease free at 8 years

ABSTRACT

Renal hydatid disease is uncommon and accounts for 2-3% of all reported cases of hydatid disease. The treatment of choice is surgical. Medical therapy is not always satisfactory, and cure depends mostly on removal of all affected tissue. Cystectomy, nephrectomy, or cystectomy with pedicled omentoplasty, especially if the cyst communicates with the renal collecting system, are the treatments of choice. The use of a scoilicidal agent is mandatory during surgery. Mixed echogenecity on ultrasound and multivesicular cyst with mixed density on computed tomography [CT] are most helpful in establishing the diagnosis. We wish to report 10 cases of renal hydatid disease encountered and successfully treated by surgery at King Hussein Medical Center [KHMC]. None of the patients had liver or lung involvement

ABSTRACT

In an endemic area for schistosomiasis in the northeast of the state of Minas Gerais in Brazil 516 individuals have been submitted to clinical and laboratory examination, ultrasonography of the abdomen and dopplerecocardiography in order to define the morbidity of schistosomiasis before and after treatment. A high prevalence of schistosomiasis (66.3 per cent) and of severe disease (9.5per cent with palpable spleens) were recorded. Ultrasonography classified liver periportal fibrosis as light (19.4 per cent), moderate (27.6 per cent) and intense (6.8 per cent), and 46.0 per cent presented no periportal fibrosis. Twenty one out of the 53 individuals (39.6 per cent) with palpable spleens did not present liver fibrosis on ultrasound. Periportal lymph nodes were described in 33.8 per cent of the population and anti-KLH antibodies were found in the serum of 40.7 per cent. Urinary alterations compatible with the glomerulopathy of schistosomiasis were observed in 4.5 per cent of the population, and 11.7 per cent of the individuals examined by dopplerecocardiography had pulmonary hypertension. Twelve months after treatment for schistosomiasis the prevalence of the disease dropped from 66.3 per cent to 25.0 per cent. In Queixadinha, a profile of the morbidity of schistosomiasis has just been established.

Strongyloides infection is usually asymptomatic but disseminated infection is a cause of morbidity and mortality in immunodeficient patients. Two fatal cases of strongyloidiasis are reported here, with rare manifestations of gastric ulcers with hematemesis and acute renal failure respectively.
